What Information Is Needed Before Requesting a Certificate?
Gather your request details and confirm payroll readiness before requesting a certificate.
|
What Details Should I Have Ready?
The certificate holder is the person or organization asking for proof of your business’s coverage. Their details belong in the certificate holder section of the request, separate from your company’s information.
Your company’s information
- Company name
- Company phone number
- Name of the person requesting the certificate
- Date of the request
Certificate holder’s information
- Certificate holder’s name
- Complete mailing address, including street, city, state, and ZIP code
These are the required details listed on SPLI’s published certificate request form. Also gather your company’s email address and the certificate holder’s contact name, email address, fax number, and phone number, as available.
Use the holder’s name and address as provided in their request. A contact person’s name or the project location may be different from the organization that needs the certificate. If you’re unsure which details to use, ask the holder before submitting.
What if the Request Includes Special Project Requirements?
If the certificate holder has provided special requirements, include those instructions in writing with your request.
For a waiver of subrogation request, SPLI’s published form also requires:
- Project name
- Project address, including city, state, and ZIP code
- Project start date
- Scope of work
If you do not understand the requested wording, contact us, and an Account Executive can help you with any questions before submitting your request. A certificate documents existing coverage; it doesn’t add coverage.
Special requirements need review to determine what can be provided.
Does My Payroll Need to Be Current?
Certificate issuance also depends on your payroll and account status.
If you already work with SPLI, make sure your payroll reporting and payments are current. Outstanding payroll requirements may prevent a certificate from being issued even when the request form is complete.
If you are new to SPLI, your first payroll generally needs to be processed before a certificate can be issued. If you need a certificate before your first payroll, contact a SPLI Account Executive to discuss available options and their conditions.

How Do I Submit a Complete Request?
Submit your information through SPLI’s certificate request form.
Before sending it, check that the company and certificate holder details are complete, the delivery contact information is accurate, and any applicable project details and written requirements are included.
If you are an Agent submitting a request for a client, use the client’s company information and identify yourself as the requester.
A complete form submits your request for review; it does not guarantee that a certificate can be issued. Submit before the certificate is needed to allow time to resolve any outstanding requirements.
